Membership Option

Plan on being a frequent customer? You can choose a membership option for $250 per month for one child, or $300 per month for multiple children. This includes unlimited virtual visits via phone, text, or video chat, and 1 house visit per child per month. Each additional house visit costs $50 per visit per month.

• 4 house visits in 4 weeks
• 1 month of direct text and phone access
• Vital signs
• Diabetes screening
• Nutrition, fitness, mental health, healthy habits, safe social media use
• Sports physical form included

What type of services are included?

  1. Well Visits (ages 0 to 21)
  2. Sick Visits – virtual and in person when needed
  3. Chronic conditions – including ADHD and mental health
  4. Developmental evaluations (including autism)
  5. Rapid testing for strep, flu, and urine
  6. Prescriptions
  7. Coordination of care
  8. Referral to specialists when necessary
  9. Sports physicals
  10. Simple laceration repair (glue, staples)

What type of services are NOT included?

  1. Vaccines – covered by insurance or available at low cost
  2. Xrays – will coordinate low cash pay prices 
  3. Blood tests – will coordinate low cash pay prices
  4. Cost of specialty care
  5. Emergency services
